What is MyHealthCard?
Any use of this site constitutes your agreement to the above mentioned Terms and
Conditions.
MyHealthCard is a confidential and secure, internet-based
service that allows users to obtain access to and organize their own health information.
The MyHealthCard provides you with useful information about opportunities
for improving the quality of your medical care that you can share with your doctor
and health coach. The information stored by patients in their accounts is not intended
to be used by physicians or health coaches for diagnosis or treatment related decisions.
What Information is contained in MyHealthCard?
The MyHealthCard will contain only the information about your health that
you choose to enter. Keeping your information up-to-date makes the MyHealthCard
a more valuable tool for you.
How may the Health Information contained within MyHealthCard be used?
The MyHealthCard serves as both a repository of your health information as well
as a tool to inform you about opportunities to improve your health. As new data
is entered, it is scanned by the system in order to identify ways in which your
diabetes care may be improved.
In-built algorithm matches information in the MyHealthCard about your health
and treatment against a set of generally accepted clinical "best practices"
derived from the medical literature. Alert messages are then delivered to you in
order to provide you with information you can discuss with your doctor. We encourage
you to speak with your doctor about the Alerts, so you and your doctor can determine
whether any change in your medical care is warranted.
You should never rely on an Alert as the sole basis for changing your medical treatment.
Alerts are for informational purposes only and do not constitute, and are not intended
as a substitute for, medical advice. Certain Alerts may not be appropriate for your
particular medical condition because the messages are based exclusively on information
supplied by you in your MyHealthCard, which information may be incomplete. As
always, if you think you have a medical emergency, you should immediately call your

Who Has Access to my MyHealthCard?
As a result, your physician, health coach, the principal investigator for this project
and database administrator at Lion Health are responsible for administering the system
will have access to your MyHealthCard.
Lion Health will not share any information in your MyHealthCard with third parties
without your consent.
How is the Security of my MyHealthCard Maintained?
MyHealthCard employs administrative, physical and technical safeguards to maintain
the security of your MyHealthCard. We believe these safeguards are consistent
with applicable law (HIPAA) and are generally effective in preventing unauthorized
access to your information.
You also play an important role in ensuring the security of your MyHealthCard.
As part of the MyHealthCard registration process, you choose a password for
accessing your MyHealthCard. You should not share your password with anyone.
You should understand that if another person has your password they will be able
to obtain access to your entire MyHealthCard as if they were you.
May I Delete Information in my MyHealthCard?
You may delete any information contained in your MyHealthCard at any time.
May a Minor Create a Record in MyHealthCard?
A person under the age of 18 may not create his or her own MyHealthCard. However,
their parent or guardian can create and administrate a MyHealthCard for them.
How can I Discontinue Use of my MyHealthCard?
Your MyHealthCard subscription lasts for one year. You may discontinue the
use of your MyHealthCard after that by choosing not to renew your subscription.
Which Restrictions Apply to my Use “Lion Health”, “MyHealthCard” and their
associated logos (“Lion Health Marks”) are trademarks and service marks of Lion Health.
All content included on Lion Health websites, including but not limited to, text,
photographs, graphics, button icons, images, artwork and other data (the “Content”)
in any form are protected by U.S. and international copyright law and conventions.
You may not use, sell, display, distribute or reproduce the Lion Health Marks or Content
for any purpose without

Lion Health’s express written consent. Upon acceptance of these terms and conditions,
Lion Health grants you a limited license to view, download and print copies of the
Lion Health Marks and Content exclusively for your personal, noncommercial use.
You agree not to use the MyHealthCard to engage in any illegal or fraudulent
activities, or to use defamatory or obscene language, or obscene graphics.
Lion Health reserves the right to immediately terminate use of and access to MyHealthCard for any person or persons engaged in any impermissible activity, or
if you violate any of these terms and conditions.
The MyHealthCard website is occasionally unavailable due to maintenance
and as such will not be available to members during those times. Lion Health shall
not be obligated or responsible for providing access to any uploaded documents during
these times.
Which Other Legal Rules Apply to MyHealthCard?
You agree to defend, indemnify and hold harmless Lion Health and its directors, officers,
employees, agents and affiliates from and against any claims, demands, liabilities
and settlements, including but not limited to, reasonable attorneys’ fees, resulting
from or arising in connection with your violation of these terms and conditions.
This website and the MyHealthCard are provided by Lion Health on an “as is” basis.
Lion Health makes no representations or warranties of any kind, express or implied,
as to the operation of this website or the MyHealthCard. To the fullest extent
permitted by applicable law, Lion Health disclaims all warranties, express or implied,
including but not limited to, implied warranties of merchantability, fitness for
a particular purpose, title or infringement. You agree that the laws of the State
of Georgia, without regarding to principles of conflict of laws, will govern these
Terms and Conditions and any dispute that may arise between you and Lion Health or
its affiliates hereunder. You expressly agree that exclusive jurisdiction for any
dispute with Lion Health or its affiliates relating to the MyHealthCard or these
terms and conditions shall reside in the courts of Georgia.
